It looks like she probably needs to fill out an I-90 (Renew or Replace a Green Card Online). The below link is to the USCIS website for that online form,

She may need to file form I-751 (Petition to Remove the Conditions of Residence) instead of the I-90. If she married an American and needs to have conditions removed then the second link is for the I-751.

You might have to read the requirements for each form to determine which is right for your circumstance. I would suggest having a printer to print out a copy for your records.

On its expiration, you need to renew it by filing Form I-90, Application for green card replacement with the USCIS. If your green card has already expired, make sure you renew it at the earliest. Though you will not lose your permanent resident status if you do not renew it, per immigration laws, you need to carry evidence of your status (a valid, unexpired green card or temporary passport stamp). If you do not renew your expired / expiring card, you may experience difficulties in obtaining employment, benefits and on re-entry into the US from abroad.

She need to fill out Form I-190 online, print it and sign, then mail it along with her old card, and proper fee. When the immigration received it, they will send her a NOA (notice of Action), then you wait until they send you an appointment for fingerprint. Make sure you mail your documents with Return Receipt so you you know when they get it and make copies of everything you sent them. The website is below.

You have to send them by mail not fax. Make sure you follow the instruction.
