All the following forms must be filed together if you are adjusting your status. I do hope your spouse is a US citizen. if not you will not be eligible for the green card until the spouse becomes a citizen. You can still file, but it will take a while.

2. Application to Register Permanent Residence (USCIS Form I-485)
3. Biographic Information (USCIS Form G-325A)
4. Affidavit of Support (USCIS Form I-864)
5. Permission for Work Authorization (Optional) (USCIS Form I-765)
Your spouse will need a G325a too
The USCIS approved doctor will have the Medical Examination Results (USCIS Form I-693). Visit uscis.gov to get the listing of the approved civil doctors in your area. A regular doctor cannot do the examination
You only need the Request for Travel Documents (Optional) (USCIS Form I-131 if you need to travel. My suggestion do not use it. It is best

Biometric appointment, approval or denial of I-130, then interview with an officer, approval or denial of adjustment of status. That is the order in which it will go. Please note if you get a denial in the I-485 interview you will be placed into deportation as a formality. You will have 120 days in which to appeal to get adjudicated by an immigration judge
4)I864 - My Wife doesn't make 125% of poverty guide lines,Can I be joint sponsor with her and if yes what is the form we need to be submitting?
Yes, you can be a co-sponsor if you had permission to get employment in the US and file income tax. You can ask a friend or family member to be the co-sponsor on the affidavit (I-864). They must work over $35,000.00. They will need to fill out a I-864 form also and send a copy of their green card/us passport/naturalization cert., social security card, 2 pay stubs, W2, current federal income tax
5) I693 - When this Examination needs to be done ? Can I go ahead and perform examination whenever ?
This must be submitted with your adjustment application so you can do it now if you are ready to file.
